Latest announcement: at about 8 a.m. on November 2, 2020, in the construction site of b-09c (bid I) in the east area of block B-09, the starting area of phase I of oujiangkou shoal, Wenzhou, a worker was suspected to have been hit on the head by a cement block (it is suspected that he did not wear a helmet correctly), and then sent to oujiangkou Hospital of Wenzhou Second Affiliated Hospital for rescue, which was ineffective and died, resulting in a total of one death.

After the accident, according to the regulations on reporting, investigation and handling of production safety accidents (order 493 of the State Council), the "112" general object strike accident investigation team of Zhejiang Xinbo development and Construction Co., Ltd. in oujiangkou industrial agglomeration zone was established, led by the emergency ecological Bureau of oujiangkou Industrial Agglomeration zone and composed of personnel from housing and Urban Rural Development Bureau, labor union, Lingni police station and other units (hereinafter referred to as accident investigation team).

Through on-site investigation, investigation and evidence collection and comprehensive analysis, the accident investigation team found out the process of the accident and emergency rescue, found out the direct and indirect causes of the accident, identified the nature and responsibility of the accident, and put forward suggestions on the handling of relevant responsible personnel and responsible units and suggestions on accident prevention and rectification measures. 1、 Overview of accident related units and personnel

（1） Location of the accident.

The accident occurred at the construction site b-09c (bid I) in the east area of block B-09, starting area of oujiangkou shoal phase I, Wenzhou, near the intersection of nixiang South Road and oucai road. The construction project was started on October 18, 2020, with a project scale of 146000 m2 and a contract price of 210.17800 yuan. The accident was in the basement foundation stage at the time of occurrence.

（3） Accident related personnel

Mu Shifei, male, Han nationality, born in February 23, 1992, address: Cha Wan Group, Tian Ba village, Tai Po Township, Xishui County, Guizhou province. The ID number is 522132199202235416. The pile breaking worker died in the accident.

2、 Accident process and rescue

At about 7:45 a.m. on November 2, 2020, Mu Shifei, a pile breaker, was carrying out pile breaking operation in the construction site of b-09c (bid I) in the east area of block B-09, phase I starting area of oujiangkou shoal, Wenzhou. He was accidentally hit on the head by the dumped pile head (it is suspected that he did not wear a safety helmet correctly). After discovering it, the nearby workers immediately dialed 120 emergency telephone and then sent to the hospital of oujiangkou new area (the Second Affiliated Hospital of Wenzhou Medical University) the rescue was declared invalid and died at 9:45.

3、 Casualties and direct economic losses caused by the accident

One person was killed in this accident, and the expenses for personal injury and death, aftermath treatment and other expenses totaled about 1.5 million yuan.

4、 Cause of accident

（1） Direct cause

Mu Shifei, a pile breaking worker, has weak safety awareness, inadequate awareness of risk factors in the operation area, low vigilance, carelessness, violation of operating procedures, entered the area where pile head dumping may occur, and was injured by the accidentally fallen pile head, which is the direct cause of the accident.

（2） Indirect cause

Zhejiang Zhonglin construction labor Co., Ltd. failed to perform its safety production responsibilities, the on-site management of the labor personnel, and the production education and training of the labor personnel, resulting in the operators' unclear understanding of the post risk and weak safety awareness in the process of pile head removal, entering the dangerous area in violation of regulations, avoiding being injured by the pile head in time, which is the indirect cause of the accident one of.

5、 Nature of accident

After investigation, it is determined that this accident is a general production safety responsibility accident.

6、 Identification of accident responsibility and handling opinions on the person responsible for the accident

1. Mu Shifei, with weak safety awareness, illegally entered the area where pile head dumping may occur and was injured on the head. In view of his death, he was exempted from relevant responsibilities.

2. Zhejiang Zhonglin construction labor Co., Ltd. failed to provide safety education to employees and lax on-site management of employees, resulting in insufficient safety awareness and prevention ability of employees, and failed to timely detect and stop employees' high-risk violations. The punishment suggestions are as follows:

The emergency management and Ecological Environment Bureau of oujiangkou industrial agglomeration zone shall file a case against Zhejiang Zhonglin Construction Labor Service Co., Ltd.

3. Dong mengxu, the legal person of Zhejiang Zhonglin construction labor Co., Ltd., failed to perform the safety production management responsibilities according to law, failed to improve the safety production responsibility system, failed to put in place the safety production education and training for employees, failed to effectively supervise and inspect the safety production work of the unit, failed to put in place the on-site management of employees, and was responsible for the accident. 7、 Suggestions on accident prevention and rectification measures

The most important reason for the occurrence of this accident is the insufficient understanding of the potential risks in the process of pile breaking operation, and there is carelessness. Zhejiang Zhonglin Construction Labor Service Co., Ltd. should seriously learn lessons, take warning, strengthen prevention, put an end to such accidents, and implement the following accident prevention and rectification measures.

1. Carefully analyze the causes of the accident, deeply draw lessons from the accident, practically strengthen the safety production management, and further implement the main responsibility of the enterprise. The general contractor shall strengthen the safety supervision of all subcontractors, improve the rules and regulations and protective measures for various production operations in the construction process, strengthen the pre job education and training and technical disclosure for operators, and strengthen the construction of the project Overall planning and coordination on the construction site.

2. Strengthen work safety publicity, education and training, and earnestly study and implement the work safety law And other relevant laws and regulations on work safety, strive to improve employees' safety awareness, operation skills, emergency risk avoidance ability and consciousness of abiding by regulations and disciplines. The new production safety law has been officially implemented since September 1. It stipulates that "production and business operation entities must provide employees with labor protection articles that meet national or industrial standards, and supervise and educate employees to wear and use them in accordance with the use rules".
